Hypermobility Consulting welcomes referrals from primary care and specialty clinicians for:

• Hypermobility initial evaluation and diagnosis
• Hypermobility management and care plans
• Hypermobility care coordination and guidance

It's easy to refer patients to our clinic through our secure online form.  This is a HIPAA compliant method for referrals.

Alternatively, referrals can be faxed to our clinic.  

To expedite the process, please include the following information if available:

• Patient email address (so we can contact them to complete our online intake process)
• Signed ROI
• Pertinent medical records
