Hypermobility Consulting is a solo medical practice.  Because of the amount of time we devote to patient care, we have limited resources to handle telephone or email inquiries.  However, we have several ways to communicate with us outlined below.

New patients can complete our intake questionnaire which is linked through our website.  If your doctor has referred you to Hypermobility Consulting, we will contact you by email with the intake questionnaire.  Please check your spam folder if you do not see the email invitation.  If your referring doctor did not provide your email address, we will reach out by text or phone to get the questionnaire to you.  If you have an additional questions, reach out to us on social media or with the form below.

Referring clinicians can complete our online referral form, full information on this process is located at Referring Clinicians.  Fax and telephone options are listed below.  You will most likely need to leave a message and we will contact you when available.
