Before signing up, please review:
If your doctor referred you to our clinic, you will receive an invitation to complete our intake questionnaire. If you did not receive this invitation within 2 weeks of referral, please go ahead and complete the questionnaire linked below.
If you are self referring, please complete our intake questionnaire linked below.
In either case, we will review your information and make sure that Hypermobility Consulting is right for your situation. You will be invited to activate our Patient Portal at which time we will provide further information about completing the full intake process and getting scheduled. If we are not a good fit for your situation, we will let you know recommended alternatives.
